re: Required Annual Testing of Backflow Prevention Device
Testing is now due for the backflow prevention device located on the water pipe that supplies your property. The device is listed on the attached Aqua America Backflow Prevention Test Report as required by The Pennsylvania Code - Title 25, Chapter 109 (the Commonwealth of Pennsylvania's official publication of rules and regulations). The property owner/tenant is responsible for this device, its certification, testing and any charges resulting from the certification.
The testing is required to ensure public safety by assuring that the device is operating properly. If the test shows that the backflow prevention device is not operating properly, you must have the necessary repairs made and have the device re-tested by the certified tester. When a test shows that the device is operating properly, you are to have the attached form filled out by the tester. You must have the final test form completed, signed by the tester, and returned no later than the test due date.

TO CONTINUE SUPPLYING WATER SERVICE, WE NEED YOUR ASSISTANCE. IF THE INFORMATION LISTED BELOW IS NOT PROVIDED WITHIN THE NEXT 10 DAYS, WE WILL SHUT OFF THE WATER SERVICE AT THE ABOVE ADDRESS FOR THE FOLLOWING REASON:
• Failure to Have Backflow Device Tested or Installed
TO STOP THE SHUT OFF, YOU MUST DO THE FOLLOWING AT ONCE:
(1) Have the backflow device(s) tested and/or installed by a certified tester and/or installer. (2) The completed test(s) must be submitted to Aqua Pennsylvania before the Shut-Off date shown on the "10-Day Shut-Off" notice.
